Consultations are by referral

As in specialty clinics across Quebec, an ENT consultation requires a referral signed by a physician. Your family doctor, a walk-in clinic physician, a dentist or another specialist can write one.

For referring physicians

Send the consultation request by fax to (450) 674-9946. A complete request speeds up triage: reason for referral, duration and course of symptoms, side affected, relevant history, current medication, results already available (audiogram, imaging, blood work) and degree of urgency. The patient’s contact details — telephone number and health insurance number — should be legible.

After the request is sent

Requests are triaged according to the nature of the problem and the degree of urgency indicated by the referring physician, so waiting times vary from one file to another. If your condition worsens while you wait, go back to your physician: a request can be re-prioritised.

On the day of your appointment

Plan to bring your health insurance card, a list of your medications and any test results you have. For a child’s appointment, a parent or guardian must be present. If you cannot attend, let the clinic know as early as possible at (450) 674-3042 — the freed slot goes to another patient who is waiting.
